Resource Integration

One of the least costly ways to address problem areas within the health of your population is to take better advantage of the current resources you have in place.  At CHC Wellness, the first step in our follow-up process is reviewing the current resources the client has available.  This includes Employee Assistance Programs (EAP), Disease Management Programs (DM), free resources from their insurance carrier and any other resources the client is paying for.  In the majority of cases, these services are under-utilized and under-promoted to the employees.  By working with our clients, CHC Wellness will ensure that employees are aware of all wellness resources they have available and how they can access them.

All of these resources should also be sharing data to provide the best treatment plan for the individual and the most information for the organization to make decisions.  CHC Wellness has the ability to integrate and transfer data to and from your existing resources like EAP and DM programs.  By sharing the risk data identified, each of these resources can better contribute to their portion of the employee’s treatment plan.
