HIPAA and Privacy Policy

Security and confidentiality of Personal Health Information (PHI) is a top priority at CHC Wellness. We will never share any PHI without a participant’s permission. Each and every CHC Wellness employee abides by HIPAA (Health Insurance Portability and Accountability Act) regulations in all of our procedures and operations. A designated Privacy Officer has been assigned to insure these procedures are followed.

We realize that extensive medical, health and lifestyle information is collected and stored at our site. This data is used to generate a personal report containing an analysis of the participant's health risks and lab data with recommendations for maintenance and follow-up. This personal profile report is confidential and provided only to the participant.

Only de-identified, aggregate data will be delivered to our client companies. Special precautions are implemented in cases of poor distribution of data within a subgroup of the report.

Data transfer of PHI may be provided only at the request or approval of the participant or in the case of a medical emergency. 

The CHC Wellness site may provide links to other sites. However, no data is available to these links. CHC Wellness is not responsible for the privacy practices or the content of such Web sites.

CHC Wellness has implemented technologically sound security measures to protect the loss, misuse and alteration of the information under our control.
